Information we collect

When you place a retail order we collect your name, contact details and delivery address. We do not collect health information on this website.

If you request a Medi-Cal coverage check, that information — including your date of birth, Medi-Cal Beneficiary Identification Card (BIC) number and prescribing physician — is collected on a separate HIPAA-compliant form operated under a business associate agreement, not on this site.

We also collect basic technical information (browser type, pages visited) to keep the site working. We do not sell personal information and we do not use third-party advertising trackers.

How we use your information

Your information is used solely to verify Medi-Cal eligibility, obtain prior authorizations, process and deliver your order, submit claims to DHCS, respond to inquiries, and meet legal record-keeping obligations for DME suppliers.

Health information & HIPAA

As a covered health care provider, Zonwaves DME protects your health information in accordance with HIPAA. See our Notice of Privacy Practices for a complete description of how protected health information may be used and disclosed.

Sharing

We share information only as needed to fulfill your order and bill your coverage: with the California Department of Health Care Services (DHCS) and its fiscal intermediary, your prescribing physician, and delivery personnel. We require comparable confidentiality from any service provider.
