Do you bill our clinic?

No. We bill the patient's plan directly, once our Medi-Cal supplier enrollment completes. There is no charge to your practice for a referral.

What documents are required?

Four: the prescription, relevant chart notes, the face-to-face encounter note, and the item list. If a HCPCS code is missing we identify it. We never guess a code.

How long does approval take?

It depends on the plan and the item. Some items need no authorization; others need a TAR, and that timing is the plan's. You can see the current status of every referral in the portal.

How do we submit referrals?

Register your facility once, then upload through the provider portal. Your NPI is checked against the NPPES registry before the first order ships.

Can we fax instead?

Yes. Our fax is 213-912-3188. If the referral is for an order we already have, please write the reference number (it looks like ZW-260722-BEAC) on the cover sheet or the first page. A fax carries no sender information, so that reference is what tells us which patient the pages belong to. Without it we can still process the referral, it just takes a person to match it up.

What areas do you serve?

Los Angeles and Orange County. Ask us about neighboring areas. Delivery is arranged case by case.

Do you take workers' compensation referrals?

Yes. California workers' comp does not require Medicare or Medi-Cal supplier enrollment, so we accept these referrals now under our California HMDR license (#129122) and NPI 1750200416.

Who submits the RFA?

The treating physician completes, signs and sends the DWC Form RFA to the claims administrator. We can prepare the item list with HCPCS codes and quantities for your RFA. The medical decision and the signature stay with your office.

What if the request is modified, denied, or disputed?

We deliver only the items and quantities in the claims administrator's written decision. If the request is denied or liability for treatment is disputed, we do not ship.

Does the injured worker pay anything?

No out-of-pocket cost for workers' comp-approved equipment.
